exists to serve the people of St. Joseph County who do not have access to health insurance or resources for the purpose of providing treatment, education and referral.

Items to Bring:
Income information – most recent 1040 tax form SS statement or Bank statement with deposit
Four consecutive weeks of pay stubs
Unemployment information
Any other assistance
